In 2007, she obtained her Master’s in Nursing and began practicing in Fort Lauderdale in Internal Medicine. Nicole began to realize that many patents lack transportation services and depend on friends and family to provide basic needs. In 2018 this realization was brought home when a family member suffered a sudden illness and became totally dependent. Home based Primary Care was not available, which often leads to poor compliance and poor outcomes. Nicole had a desire to make an impact in this area. She later started N-Hall Medical Services where she provides quality care and offers Home Based Primary Care to Homebound patients. Nicole provides care from a wholistic approach and believes that patient care should not focus solely on the physical ailment but the multiple factors that contribute to their overall health. Raised in the same Southwest Florida community she serves, Sheeja Kanacheril, D.O. has been dedicated to caring for underserved populations throughout her career. She obtained her Bachelor of Arts from Tulane University in New Orleans, LA, graduated from Nova Southeastern University College of Osteopathic Medicine in Fort Lauderdale, FL in 2004, and completed her family medicine residency at the Duke/SR-AHEC program in Fayetteville, NC before returning to her home state of Florida. Dr. Kanacheril completed four years with the National Health Service Corps in health profession shortage areas and worked for Lehigh Medical Group as a primary care provider before joining Hope Healthcare in 2013. She was an instrumental part of the Hope PACE program for 5 years, practicing center- and home-based, all-inclusive geriatric care in a three-county area. In 2018, Dr. Kanacheril became Associate Medical Director for the Hope Hospice program and team physician for two home teams serving predominantly rural areas. She received her Masters of Nursing degree from Gulf Coast University and is a member of Sigma Theta Tau International. She now works with one of the largest ACO companies in the nation, Millennium Physicians Group, doing their home based primary care visits. Millennium spans the entire state of Florida. She actively works with the physicians within the company to prevent hospitalizations and unnecessary ER visits.
