Home Centered Care Institute (“HCCI,” “we,” “us,” or “our”) are committed to protecting the privacy of all individuals who utilize the HCCI site. HCCI is the data controller of the personal data collected, and is responsible for the processing of your personal data. This Privacy Policy is intended to inform you of our policies and practices regarding our collection, use, and disclosure of the information you submit to us and that we collect from you through the website at www.hccinstitute.org and any other site controlled by HCCI (collectively referred to as the “Site”). This Privacy Policy does not govern information you might provide through a channel other than this Site. This Privacy Policy applies to all visitors to the Site.
Please review this Privacy Policy, which is incorporated into and made part of our Terms of Use. It is important that you read this Privacy Policy together with any other notice HCCI may provide when we are collecting or processing your personal data so that you are fully aware of how and why we are using your data. This Privacy Policy supplements other notices and is not intended to override them. In the event of a conflict between this Privacy Policy and another notice which applies to a specific circumstance, the latter shall prevail over this Privacy Policy to the extent of the conflict.
This Site is not intended for the collection of protected health information. As such, please do not provide HCCI with protected health information through this Site without providing express written consent to disclosure. If you are a patient receiving medical care from a healthcare provider, you may have other rights with respect to the access, use, and disclosure of your protected health information.
Please review the following to understand how we at HCCI process and safeguard personal data about you. By using the HCCI Site or otherwise contacting HCCI or and/or by voluntarily providing personal data to HCCI, you acknowledge that you have read and understand the practices contained in this Privacy Policy and consent to our collection, use and disclosure of your information in accordance with this Privacy Policy and Terms of Use. If you do not agree to our Privacy Policy or Terms of Use, you may not use the Services.
This Policy may be revised from time to time, so please ensure that you check this page periodically to remain fully informed. HCCI reserves the right to change this policy without notice. If you use the Site after we post changes to this Privacy Policy, you accept and agree to be bound by the changed policy.
Information HCCI Collects And How HCCI Uses It
You may choose to provide personal information (for example, your name, address, email address, telephone number) on our Site for various purposes, including the following.
Personal Information You Provide
- Newsletter: You may use the Site to sign up for our newsletter, which requires your email address, name, phone number, mailing address, job title, and company. HCCI will use this information to send you the communications you requested.
- Donations: You may make donations to HCCI through the Site and will be asked for your name, mailing address, phone number, donation amount, and payment method information (such as credit or debit card number, or bank account information).
- Education: You may use the Site to watch webinars by setting up an account through HCCI’s Learning Hub. In setting up an account, you will be asked to provide information such as your name, email address, mailing address, contact phone number, and company information. HCCI may use this information to send you communications regarding the webinars or other educational material.
- Other site features: The Site offers features that may change from time to time. HCCI may provide you with an opportunity to communicate about these features by submitting certain personal information.
- Other uses of information: HCCI may use the personal information you provide on the Site for internal purposes. These purposes include administration of the Site, market research, data analytics, and compliance with our legal obligations, policies, and procedures.
Email: HCCI only sends emails to users of our Site who sign up for HCCI mailing lists or who agree to receive future communications from us. Every email you will receive from HCCI will contain clear information on how to unsubscribe from future communications. If you sign up for communications from HCCI but then wish to unsubscribe, follow the unsubscribe instructions in the email you receive. Upon receipt of any unsubscribe request, HCCI will cease our communication with you until and unless you re-subscribe for one of our mailing lists.
Telephone: HCCI may also contact you by phone where you provide your phone number, in connection with the request, comment, or inquiry you submit via the Site. If you do not wish to be contacted by phone, please indicate that to the individual who is calling you on our behalf, and HCCI will remove your phone number from the HCCI contact list.
Cookie Policy – Personal Information We Collect By Automated Means
HCCI may collect certain information by automated means when you visit our Site, such as how many users visit our Site and other related analytics. HCCI collects this information to learn how to improve the HCCI Site and how to best tailor it to HCCI visitors. When you visit the Site, HCCI may automatically collect some information about your visit using cookies, log analytics software, and other aggregate tracking technologies. Cookies are bits of text that are placed on your computer’s hard drive when you visit certain websites. HCCI may use cookies to tell us, for example, whether you have visited HCCI before. If you wish, you may decline cookies or set your browser to refuse cookies. Most browsers will tell you how to stop accepting new cookies, how to be notified when you receive a new cookie, and how to disable existing cookies.
- Analytics: HCCI may allow others to provide analytic services on our behalf, which may use cookies, web beacons, and other technologies to collect information about your use of our Site, including your IP address, web browser, pages viewed, time spent on page, linked clicked and conversion information. We may use analytic partners to help us analyze and track data, determine the popularity of certain content, deliver advertising and content targeted to your interests on the Site, evaluate your use of the website, complied reports on website activity, and better understand your online activity. This information may be transmitted and stored by the analytic partner on servers that are not controlled by HCCI. The analytic partner may also transfer information to third parties when required to do so by law, or when such third parties process the information on the analytic partner’s behalf. - Social media: HCCI may use social media sharing and other “widgets” to enable interaction with social networks, such as Facebook, Instagram, Twitter, LinkedIn, and Youtube. Some of these social networks may be able to track your visit to our Site (or any site that uses the widgets) because when you visit a social network, it may place personally identifiable cookies in your browser that are then transmitted back to the social network when you visit other sites that use that network’s widgets, even if you do not click on or interact with the widget. If you have questions about a particular social network’s information collection or cookie practices, please refer to the privacy and use policies of that social network
- Functionality: These cookies allow our Site to remember choices you make (such as your user name, language or the region you are in) and provide enhanced features. They may also be used to provide services you have requested such as viewing a video or commenting on an article. The information these cookies collect is usually anonymized. They do not gather any information about you that could be used for advertising, or remember where you have been on the internet.
The major browsers have attempted to implement the draft “Do Not Track” (“DNT”) standard of the World Wide Web Consortium (“W3C”) in their latest releases. As this standard has not been finalized, the HCCI Site does not currently respond to or process DNT requests.
You have choices about certain information HCCI collects. When you are asked to provide information, you may decline to do so; but if you choose not to provide information that is necessary for us to collect to complete your order or respond to your request, we may not be able to complete your order or respond to your request.
How Does HCCI Process Personal Data?
Business Operations and Marketing.
- HCCI might use your information to provide you with information personalized to your requests or to respond to inquiries;
- HCCI might use your personal data for screening you for employment or other engagement;
- HCCI might use your personal data to provide you with targeted information and advertising on the site or to contact you with information and promotional materials, offers, and requests for donations;

Legal Bases for Processing. HCCI processes personal data for, or based on, one or more of the following legal bases:

- as is necessary for our legitimate interests, including direct marketing;
- to comply with the law and our legal obligations; and
- in certain circumstances, based on your express consent.
Information HCCI Shares With Others
- Some of the above processing involves sharing collected personal data with third parties, including service providers, affiliates, and other partners. HCCI will not sell, rent, trade, exchange, share, or otherwise disclose personal information visitors submit through our Site, except as identified in this Privacy Policy.
- Consent: HCCI may share information you submit to us with your consent or at your direction.
- Service providers: HCCI may share your information with service providers we have retained to perform services on our behalf. We obligate these service providers to not use or disclose the information they receive from us except as necessary to perform services on our behalf or to comply with legal requirements.
- Business Partners: HCCI may share personal data with third party business partners in connect with donations that we receive. Examples of third parties to whom we may disclose personal data for this purpose are banks and payment method providers (such as credit card networks) when we provide payment processing services.
- External links: The Site may include links to other websites whose privacy practices and policies HCCI does not control. Once you leave the Site, disclosure or provision of your personal information, if any, is governed by the privacy policy and terms of use of the owner of the site you are visiting.
- Compliance with law: In addition, HCCI may disclose information about you (i) if we are required to do so by law, (ii) in response to a request from law enforcement authorities or other government officials, or (iii) when we believe disclosure is necessary or appropriate to prevent physical harm or financial loss or in connection with an investigation of suspected or actual illegal activity.
Protection of Personal Data
Security is important to HCCI and to you. Therefore, HCCI works to employ reasonable and appropriate physical, technical, and organizational safeguards designed to promote the security of our systems and protect the confidentiality, integrity, availability, and resilience of personal data. If you suspect unauthorized access to your information as a result of a breach of HCCI protective measures, you should contact HCCI immediately.
No method of safeguarding information is completely secure. While HCCI uses measures designed to protect personal data, HCCI cannot guarantee that any safeguards will be effective or sufficient. Internet data transmission is not always secure, and HCCI cannot warrant that information you transmit to us through the Internet is or will be secure.
Retention of Personal Data.
HCCI retains personal data for as long as is necessary to fulfil the purpose for which it was collected, as well as to the extent HCCI deems necessary to carry out the processing activities described above, including compliance with applicable laws, regulations, rules and requests of relevant law enforcement and/or other governmental agencies, and to the extent HCCI reasonably deems necessary to protect the rights, property, and safety of HCCI, our customers, and other third parties.

- the right to be informed about the personal data HCCI collects and/or processes about you;
- the right to access, modify, and correct personal data about you;
- the right to know with whom HCCI has shared personal data about you, for what purposes, and what personal data has been shared (including whether personal data was disclosed to third parties for their own direct marketing purposes);
- where processing of personal data about you is based on consent, the right to withdraw your consent to such processing; and

Accessing, Modifying, Rectifying, and Correcting Collected Personal Data. HCCI strives to maintain the accuracy of any personal data collected from you, and will use commercially reasonable efforts to respond promptly to update the HCCI database when you tell us the information in our database is not accurate. However, we must rely upon you to ensure that the information you provide to us is complete, accurate, and up-to-date, and to inform us of any changes. Please review all of your information carefully before submitting it to us, and please notify us as soon as possible of any updates or corrections.
In accordance with applicable law, you may obtain from us certain personal data that we process about you. If you wish to access, review, or make any changes to personal data you have provided to us through our Site or related services, you may do so at any time by contacting us as provided below. Please note, however, that we reserve the right to deny access as permitted or required by applicable law.
Children’s Information
The Site is not directed at children, and HCCI will not knowingly accept, request, or collect personal information from individuals under the age of 18. If we learn that we have collected personal information from an individual under the age of 18, we will delete this information from our databases in accordance with our deletion procedures. Should you suspect that a child of whom you are the parent or legal guardian has provided HCCI with personal information, please contact us and we will work with you to address this issue.
Your California Privacy Rights
Under California Civil Code Section 1798.83, California users are entitled to request information relating to whether a business has disclosed personal information to any third parties for the third parties’ direct marketing purposes. HCCI does not sell or share your personal information with third party companies for their direct marketing purposes without your consent.
Additional Information for Individuals in the European Economic Area (“EEA”)
The information contained in this Privacy Police and the Terms of Use applies to any individual located in EEA. Under the European Union’s General Data Protection Regulation (“GDPR”), in certain circumstances, an EEA-resident has certain individual rights with respect to personal data that we hold about them. In particular, you may have the right to:
- Be informed; This means anyone processing your personal data must make clear what they are processing, why, and who else the data may be passed to;
- Request access to any data held about you;
- Ask to have inaccurate data amended;
- Request data held about you to be erased, provided the data is not required by HCCI to perform a contract, protect its rights, interests or those of a third party, defend against a legal claim or to comply with applicable laws or regulations;
- Prevent or restrict processing of data which is no longer required;
- Request transfer of appropriate data to a third party where this is technically feasible;
- Object to further processing of data which is inconsistent with the primary purpose for which it was collected; and
- Not be subject to a decision based solely on automated processing.
Additionally, in the circumstances where you may have provided your consent to the collection, processing and transfer of your personal data for a specific purpose, you have the right to withdraw your consent for that specific purpose at any time. Once we have received notification that you have withdrawn your consent, we will no longer process your information for the purpose or purposes you originally agreed to, unless we have another legitimate basis for doing so in law.
HCCI acts as the “Data Controller” under the GDPR. To exercise any of these rights, please contact us using the contact details set out under the “Your Choices” heading below. As a resident of the EEA, you are also entitled to direct any complaints in relation to our processing of your personal data to your national or local data protection authority.
Changes to Policy
From time to time, HCCI may change this Privacy Policy. HCCI will post a prominent notice of any material changes to the Policy on the Site, and indicate at the top of the notice when it was most recently updated. Your use of our Site following any such change constitutes your agreement to follow and be bound by the Privacy Policy, as amended.
