The Growing and Urgent Need for Home-Based Primary Care in Illinois

Why Illinois? *

  • Illinois is home to 2.1 million older adults, and at least 40% are 75+ years of age and 12% are 85+.
  • Illinois has more than 700,000 residents who have ambulatory difficulties, and 40% of Medicare beneficiaries across the state have 4+ chronic conditions.
  • Illinois has approximately 260,000 individuals who are homebound, with only 26% of these receiving home-based primary care (HBPC).

Why Is it important?

  • Because the median age in Illinois has gone from 37.2 to 39.2 in the last 10 years and is going to keep increasing as our society ages.
  • Because older adults managing serious illness or facing significant mobility issues benefit from home-based care.
  • Because Illinoisans have the right to receive care in the place of their choosing
  • Because HCCI is addressing the problem of limited access by educating, training, and growing practices across the state.
