Thanks to a grant from the RRF Foundation for Aging, HCCI’s Florida House Call Project has identified, equipped, and motivated a network of home-based primary care providers to spread awareness and promote the benefits of HBPC.

The Home Centered Care Institute is increasing access to home-based primary care through education, consulting, research, and advocacy because the future of healthcare is in the home.
Known as the “modern-day house call,” Home-Based Primary Care (HBPC) is exactly what it sounds like – physicians, nurse practitioners, and physician assistants travelling to a patient’s home, or the place they call “home,” to deliver primary care services.
HBPC dramatically improves health outcomes and the quality of life for homebound patients and their caregivers. Providing ongoing in-home care to frail, medically-complex individuals also reduces the overall cost of care by reducing unnecessary emergency room visits and hospitalizations.
Currently, over 7 million people in the U.S. need HBPC, but only about 15% of them can access it. As a nationally recognized authority on clinical and operational best practices for home-based medical care, HCCI is an ideal resource partner for individuals, organizations, investors, and healthcare systems considering starting or expanding an HPBC practice – which can help address this access gap.
