About Dr. Li
Li has provided leadership for ChristianaCare’s Geriatrics, Supportive and Palliative Care, Home Health, and other post-acute initiatives since 2006. She developed and continues to help grow ChristianaCare’s House Call Program which encompasses 13 providers who deliver care to 900+ patients.
Li holds an undergraduate degree from Princeton University and completed her medical training at Rutgers University. She is board-certified in family medicine, geriatric medicine, and pain and palliative care, and a Clinical Associate Professor for the Department of Family and Community Medicine.
During HCCI’s Essential Elements of Home-Based Primary Care™ workshop, Li presents patient case studies and topics that include “What Matters: Prognostication and Advance Care Planning in HBPC”; “Medication: Aligning Treatment with the Specific Needs of Frail Older Adults”; “Safety Strategies for House Call Providers”; and “Supporting Caregivers of Homebound Patients.”
How did you get involved with house calls?
After my residency training, I completed a geriatric and clinical-research fellowship which sparked an interest in home-based care. Then in 2006, I joined ChristianaCare to head up their geriatrics practice and home care program. At the time, there was one other provider doing house calls. Now we have 13 providers serving 900+ medically complex homebound patients.

What’s your most memorable HCCI workshop moment?
I remember standing outside after a session waiting for a shuttle when a learner approached me. She said her boss had encouraged her to attend and she hadn’t known what to expect but was so inspired by the entire experience—and that every session was top-notch. Feedback like that is priceless.
