HCCI Essential Elements of Home-Based Primary Care™ Workshop
Course Description
This two-day foundational workshop is designed for learners who are considering or who are relatively new to offering home-based primary care (HBPC) services for patients with complex medical conditions. Through this workshop, clinical providers, practice managers and operations staff will enhance their knowledge, competence, and performance in providing optimal HBPC based on current recommendations from national experts. This classroom workshop features case-based sessions focusing on clinical and economic factors unique to home-based primary care, including patient assessment; infection control; medication management; acute/urgent care; technology useful in enhancing patient care and productivity; cultural competence; staffing, operations and durable medical equipment; coding and billing; provider safety and self-care.

HCCI Advanced Applications of Home-Based Primary Care™
The HCCI Advanced Applications of Home-Based Primary Care™ is a 2-day workshop designed for clinical providers and practice managers/operations staff with some experience in providing home-based primary care (HBPC) services and who are seeking to advance their knowledge, skills, and productivity to provide this care for patients with complex medical conditions. This workshop is also ideal for those who have recently completed the HCCI Essential Elements of Home-Based Primary Care™ workshop and are ready to continue their learning.
The HCCI Advanced Applications of Home-Based Primary Care™ workshop is targeted to learners interested in benchmarking their practices against those recommended by national experts. This classroom workshop features case-based sessions focusing on advance care planning, communication, and prognostication; care coordination and transitions of care; caregiver support; managing challenging clinical issues in HBPC, including dementia, polypharmacy, acute/emergent situations, and wound care; coding, documentation, and billing for services beyond E/M; staffing models, workflow/scheduling, and productivity; and value-based partnerships.
Each participant will receive two days of classroom training taught by HBPC industry experts affiliated with one or more of our Centers of Excellence (COE) – leading institutions of higher medical learning around the U.S. Workshop enrollment also provides access to an exclusive HCCI University online discussion forum
and supplemental workshop information.
