The Home Centered Care Institute (HCCI) is dedicated to creating universal access to best practice house call programs, making home-based primary care the national standard for treating medically complex patients who are better cared for in the home.
We seek to ensure every patient in need has access to quality medical care in the home that results in a profound improvement in medical outcomes, patient and family experience and health care spend.

Introduction to Home-Based Primary Care and HCCI
Home-based primary care (HBPC), also known as the “modern-day house call,” provides medical care to frail, medically complex patients who are either homebound or home-limited. These are patients for whom visiting a doctor in their office is either difficult or impossible. Physicians, nurse practitioners, and physician assistants provide ongoing care to these patients in their homes – often through to the end of their lives. Thanks to advances in modern technology, a visit can even include lab tests, an EKG, x-ray, ultrasound, and more. House calls have not only been proven to dramatically improve health outcomes and the quality of life for patients and their caregivers, but also can reduce the overall cost of care by reducing unnecessary emergency room visits and hospitalizations.
The Home Centered Care Institute (HCCI) is a national nonprofit organization focused on scaling home-based primary care and bringing it into the healthcare mainstream. In addition to being an authoritative source for clinical and operational best practices for house call programs, HCCI is advancing the field through education, consulting, research, and advocacy.

Rethinking Modern Medicine
The Home Centered Care Institute’s video, “Rethinking Modern Medicine,” highlights what happens during an initial home-based primary care (HBPC) visit, including the patient assessment, exam, and conversations with the caregiver. At the same time, it shares the feeling of house calls, as they help people feel cared for and connected.
Known as the “modern-day house call,” HBPC is exactly what it sounds like — physicians, nurse practitioners, and physician assistants traveling to a patient’s home, or the place they call “home,” to deliver primary care services.

Welcome to the website of the Home Centered Care Institute (HCCI), a not-for-profit organization dedicated to expanding house call practices throughout the nation. Our work focuses on bringing you the knowledge, perspective and experience of leading experts in the field, and educating and guiding you and other professionals in the rewarding practice of home-based primary care (HBPC).
As a practicing physician with over 30 years of experience, I have dedicated my career to HBPC, personally making over 33,000 house calls. I also understand the important difference home-based primary care makes in patients’ lives and the value it brings to the health care industry. This is one of the many key reasons I founded this organization and passionately support the growth of HBPC in our communities.

Q&A with HCCI President Julie Sacks
What is your vision for HCCI?
When I think of our vision, I think of what we often say at HCCI, which is that the future of healthcare is in the home. We are working to realize that vision through four key strategies: education, consulting, research, and advocacy. We have already done significant awareness-building around the importance of home-based primary care, but there is much still to do. With HCCI’s dedicated staff, consultants, and faculty, we can make great strides in increasing the availability of home-based care.
Our research shows that the demand for home-based primary care far outweighs the supply. Patients want and need this care, but not enough providers deliver it. We are looking at how we can add to that supply through workforce development and by encouraging practices, investors, and payers to get behind it. My overarching goal is to improve equity by improving access to high-quality care delivered in the home.
What is the biggest opportunity and challenge for home-based primary care?
One of the biggest opportunities is the shift from a fee-for-service model to value-based care. Home-based primary care providers can ultimately improve patient outcomes and save money by engaging in value-based payment arrangements. Health Care’s Perfect Storm
